The City of Richmond has released the preliminary after-action report into the widespread water crisis that sent shockwaves through the region in January. It details several revelations on how the crisis took place — including that the plant may not have lost power if a component had been properly connected.

The initial findings from consultant HNTB Corp. follows similar reports released earlier this week by Henrico and Hanover counties, which are partly served by the plant and also experienced service disruptions.

Hanover leaders reviewed the findings of a third party investigation into actions taken by the county following January's water crisis that left residents without water and under a boil water advisory.
